How to get an Excel form to allow multiple answers and record them separately?

I am making an Excel form to record testing results. I made one last year and it worked fine, but it was clunky, this year I'd like to update it and get it working more smoothly.
The data I'm collecting are whether or not an allergen test passes. We have multiple buildings, each with multiple rooms, and then multiple pieces of equipment inside each room. When tests are needed they only happen in one room at a time, but multiple pieces of equipment will be tested.
I would like to find a way that we can start the form and answer questions about the date and which building and room, and then have a question about the equipment that allows you to select multiple answers. Then I want to be able to answer pass or fail for each selected equipment.
Theoretically I could brute force it, and make a separate question on passing or failing for each and every piece of equipment, but that would be a ton of questions and I'm not sure how well it would translate to a chart for trending purposes. Last year I just had the form only accept one answer, so to submit all the results for a room where 5 pieces of equipment were swabbed, they would have to submit the form 5 times.
I would like a more streamlined solution, if there is one.

I've tried to pare it down as much as possible for those who have to do the actual data entry, but they are still very overwhelmed by it (they really don't understand how to even use filters to narrow down the scope) and so the data coming out is not reliable. I've never used MS forms in conjunction with excel, but I'm thinking that if I create a form for them just to enter the basics, I would at least have correct data for my analysis piece. The only thing I'm concerned about is that there are updates to entries that need to be made (ie- someone earns an additional level of certification or they drop out of the program). Can this be accomplished with forms? Or am I barking up the wrong tree. I was hoping to get some advice before I start really teaching myself how to use MS forms (any suggestions for good tutorials would be great too). - Help & Advice: I need to make an excel spreadsheet linked to a form that randomly selects ~11 responses/rows & moves those responses to a different tabI am working for a free healthcare clinic, and I desperately need help with excel. For our clinic, interested patients fill out a microsoft form, and then they are randomly selected to be seen by us in the excel spreadsheet linked to the form. - Optimizing data entry in Excel file on SharePoint w/ 5 users: Is MS Forms + Power Automate the way?I want to optimize data entry for an Excel spreadsheet stored on SharePoint, used by 5 data enterers. It has around 300 entries/rows and 17 columns on the main worksheet. 4 other worksheets contain additional variables/columns for the same entries on the main worksheet. 8 other worksheets contain extensive instructions for the systematic literature review we are conducting. Edit: Our current approach has each of us open the Excel file in our desk top app for data entry. The issues I want to solve are: Improved data entry environment: 1) Some cells end up with extensive/long text entries. Users resize cells to optimize their view, but that screws up other people's views. We need data entry solution that allows large data entry areas for certain Excel cells, but not others. 2) Decrease users mistakenly entering data in wrong cell. formatting issues caused by each user doing their own rogue formatting (Despite discussing not messing with formatting in couple of meetings, these people, led by our boss, keep doing it!) excel sheet freezing for short periods of time mouse pointer highlighting cells above the cell to which it is pointing A bit of Googling led me to MS Forms + Power Automate. Would this solve the issues described above and be stable. Is there a better solution? - Totaling responses from a field?This is a pretty beginner question I think but my brain is fried today. I have a spreadsheet with 3 columns. The first two I don't need at all. The last one is the filled with answers to "How did you find us" on our checkout page. I want to make a graph or pie chart that totals all the answers and shows the most common responses by percentages or count. The column's data has several pre-filled answers they can select, but they can also write their own so there's tons of custom responses. I don't have a way to reformat it so each response is in it's own column which I think is what I need to do to properly display it?
