Finding Stenoses in Angiograms: Why Recall Outweighs Perfection

Detecting candidate stenoses in coronary angiograms means prioritizing recall over perfection. False positives are acceptable; flagging half the image is not. You've already seen YOLO succeed on one dataset and fail on another, largely because of inconsistent annotation formats. Vessel segmentation and centerlines offer a smarter path: build a radius profile to flag narrowings, then filter candidates. The real challenge is evaluation. Doctors don't mark every narrowing, so your recall metrics will be too optimistic.
